Job summary Surrey Downs Health & Care The Rehabilitation Support Worker role will function as part of the inpatient team providing care, support and rehabilitation within a community setting to provide a seamless service and to support discharges from hospital. The post holder will undertake a range of delegated tasks from patient exercises and rehabilitation programmes to facilitating group work and community follow up visits of patients once discharged following a care plan and will report to the team lead/s. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care. The role will primarily sit within the therapy team but will sit between the therapy and clinical team, enabling cross cover when required. It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical teams with focus on clients' need.
