We are seeking a motivated and experienced pharmacist to work directlywith multidisciplinary teams to improve clinical effectiveness andperson-centred outcomes.The successful candidate willplay a key role in medicines optimisation, clinical medication reviews, and thesafe and effective use of medicines across the practice population.
The practice population is diverseand varies in demographics, so the breadth of health care need is wide. Thismakes working in general practice interesting and no day is the same.The role will be based within the practiceproviding support to the GPs and other healthcare professionals within theteams to improve the health of patients through the rational and safe use ofmedicines.
The post holder will contribute to the efficient workingof the practice team by leading on key areas in relation to medicinesoptimisation. The role will include face to face and remote patient contact, aswell as liaison with colleagues in community pharmacy, care homes and secondarycare.
Main duties of the job
* To develop and facilitate a good workingrelationship with all local providers of healthcare.
* To liaise effectively with other health careteams concerned with the patient care as appropriate and with all otherdisciplines within the practice.
* To plan and organise own workload, includingaudit and project work, and training sessions for members of the medicinesmanagement team, practice team, community pharmacy team, community nurse team,patients, carers, etc.
* To ensure that all patient informationfollowing consultations, and telephone encounters are recorded accurately onthe computer.
* To maintain registration as a pharmacist andcomply with appropriate professional codes.
* As appropriate to the post, to maintain anddevelop professional competence and expertise, keep up to date withmedical/therapeutic evidence and opinion, and local and national service,legislation and policy developments, agree objectives and a personaldevelopment plan and participate in the appraisal process.

About us
The Borchardt Medical Centre is a friendly and busy GP Practice located in Withington, Manchester. We have approximately 10,200 registered patients. All the staff are friendly and we have a supportive working environment where everyone is approachable.
We are using EMIS Web and DOCMAN as our clinical system along with Accurx to provide digital platform to our patients
There is staff car parking available on site.
Job responsibilities
Conduct structured medication reviews (SMRs) for patients with complex polypharmacy, frailty, or long-term conditions.
Manage and monitor long-term conditions (e.g., hypertension, asthma, diabetes) under appropriate clinical supervision.
Provide face-to-face, telephone, and digital consultations.
Support safe deprescribing and medicines reconciliation following discharge or clinic visits.
Implement national and local guidelines to ensure evidence-based prescribing.
Undertake clinical audits and quality improvement initiatives.
Identify and manage medicines-related risks, safety issues, and MHRA alerts.
Support antimicrobial stewardship initiatives.
Review repeat prescriptions, ensuring accuracy, appropriateness, and compliance.
Oversee repeat prescribing processes and medication synchronisation.
Support medicines optimisation, cost-effective prescribing, and formulary adherence.
Assist with shared-care agreements and high-risk medication monitoring.
Person Specification
Qualifications
* Registered Pharmacist with the General Pharmaceutical Council, GPhC.
* Evidence of continuing professional development CPD.
* Minimum of 2 years post-qualification experience
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
* An appreciation of the nature of primary care and the diversity that exists between general practices
* An appreciation of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
* Can plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions
* knowledge of Emis Web
* Worked in General Practice
* Independent prescriber or working towards gaining independent prescribing qualification
