To support this mission, we are expanding our team and are excited to offer a fantastic opportunity for Registered Social Workers with CHC experience to support remote Continuing Healthcare (CHC) reviews and assessments. This is a unique opportunity for career growth, allowing you to enhance and utilise your clinical, assessment, and leadership skills, all while enjoying the opportunity of working from the comfort of your own home., While working within the National Framework for NHS Continuing Healthcare (CHC) and/or the Care Act 2014, you will attend Multi-Disciplinary Meetings (MDTs) to support completion of the Decision Support Tool (DST) and conduct reviews and assessments on behalf of Integrated Care Boards (ICBs) and Health Boards. These assessments will be carried out with the individual, their provider, and family (or their representatives), all conducted virtually via MS Teams or Zoom.
