We are looking for a compassionate and motivated individual to work as an integral member of the specialist gynaecology cancer care team, providing support to the cancer clinical nurse specialists to ensure patients receive personalised care from the point of a cancer diagnosis throughout their pathway. This role offers a fantastic opportunity to work with patients and positively impact their experience of care by supporting them throughout the challenges a cancer diagnosis can present.


Main duties of the job

The post holder will support the implementation of Personalised Care for people affected by cancer including:

o Working with the clinical nurse specialists when implementing Personalised Care Interventions including:o assisting with and/or delivering holistic needs assessments and producing associated care and support planso Personalised Stratified Follow-up, i.e. risk-stratified remote monitoring and surveillanceo Provide a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needso Communicating effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, e.g. patients, relatives, carers, other health/social care professionals and community workers.o Providing health and well-being information and support in relation to their cancer, signposting to supportive services to enable navigation of health and social care; to empower the patient to manage their cancer.o Promoting equity for patients across Stockport.The post holder will be expected to work with all grades of medical and nursing staff, wider health / social care colleagues and community teams (including third sector and charity).

Job responsibilities

Cancer Patient Tracking To work with the Cancer Access Manager and Cancer Services Manager to ensure national and local cancer targets are met, potential breach patients are highlighted and actions are taken to ensure breaches are avoided. Provide detailed breach analysis reports for any patient with waiting times outside of national standards within 5 days of the date of first treatment. Appropriate transfer and tracking of patient undergoing diagnostics or treatment at tertiary sites To collect and enter data at all stages on the suspected/confirmed cancer pathway for given tumour groups and record this information on the SCR database, ensuring, through liaison with the clinical MDT teams, that this data is accurate, timely and complete. Ensure data collected enables the full and accurate production of the PTL (Patient tracking list) To follow the Trusts Cancer Targets Escalation SOP highlighting issues to the relevant contacts in the required timescales.To ensure that exceptions and alterations to the waiting times of individual patients are appropriately supported by documentation and can be audited. Work alongside and in conjunction with the other Cancer Pathway Coordinators to provide cover and share best practice to enable continuous service even during periods of sickness or annual leave.

Responsibility for Patient Care Negotiate with all involved in cancer issues to ensure appropriate management of patient information and coordination of patient pathways. To alert high risk patients (potential breaches) to Cancer Access Manager and Cancer Services Manager. Establish and ensure the continuation of effective communication within Cancer Services and other Hospital Divisions. To be responsible for the receipt and dispatch of the Communication and Referral Proforma ensuring these are processed within local and national deadlines. To telephone patients regarding follow-up appointments, encouraging timely attendance and compliance with appointments.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require a Standard DBS check at a cost of £26.50. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.


Person Specification


Education & Qualifications

* ECDL or equivalent computer literacy
* Administrative / office experience


Experience

* Understanding of medical terminology
* Experience of using / updating databases
* Experience of using NHS systems eg PAS, Somerset


Skills and abilities

* Experience of forming good mulit professional relationships


Knowledge

* Knowledge of cancer waiting time standards
