As the Care Coordinator is responsible for organising, monitoring, and maintaining the smooth delivery of care services. You will be the link between clients, carers, and management – ensuring that care is planned, scheduled, and delivered to the highest possible standard.
Responsibilities
Care Coordination & Scheduling
* Support the management team in conducting initial assessments, care plan updates, audits and reviews.
* Create, manage, and update staff rotas to ensure continuity of care and full coverage.
* Assisting with recruiting and on-boarding of new care workers to the team, matching them to service users based on skills, training, and compatibility.
* Effectively handle initial enquiries from prospective service users and their families.
* Respond to short-notice changes, sickness, or emergencies with professionalism and efficiency.
* Coordinate appointments, referrals, and follow-up care to ensure seamless transitions between services.
* Liaise with other professionals involved in service users' care.
Client & Carer Support
* Be the first point of contact for prospective service users and the care team.
* Monitor care workers and carry out supervisions and appraisals.
* Build strong, trusting relationships with clients and families, ensuring their needs are met and exceeded.
* Support carers in their day-to-day roles, ensuring they feel valued and supported.
* Work closely with management to highlight issues to reduce staffing risks, including formulation of action plans.
* Act as a point of escalation to the carers with a confident approach in supporting solutions.
* Out-of-hours and on-call duties(on a rota basis)
Compliance & Quality
* Maintain accurate, up-to-date care records in line with CQC requirements and company policies.
* Assist with audits, spot checks, and supervision of care staff.
* Ensure all scheduling respects working time regulations, travel time, and mileage considerations.
* Ensure industry and company policies and procedures are followed by the wider team.
* Supporting the effective handling of safeguarding, incidents, and complaints as required.
* Conduct live monitoring using bespoke softwareand be the first point of contact for day-to-day operational requirements of our care team.
Job Types: Full-time, Permanent
Pay: £25,000.00-£28,000.00 per year

Experience:
* providing care: 1 year (preferred)

Licence/Certification:
* Driving Licence (required)
* NVQ Level 3 Health & Social Care (preferred)
