Overview
We need a permanent, part-time Social Worker to work across Lancashire as part of our Urgent Care Acute and Prisons Service Living Better Lives in Lancashire. This is a hugely exciting time for us. We launched Living Better Lives in Lancashire — our new strength-based vision for social care. We are driving forward innovative practice that will make a difference to our communities and we need new social workers to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home. The team provides a single point of access to community intermediate care services, including reablement and residential rehab, 7 days per week. The team supports hospital discharges and works with the wider multi-disciplinary team to prevent hospital admissions and supports people to remain at home. The team also utilises the Discharge to Assess model to ensure that assessments are done away from the hospital environment. This key role will also involve holistic, face-to-face Care Act Assessments, reviews and support planning, using a strengths-based approach to ensure that people receive the right support at the right time, enabling them to achieve their optimum level of independence and live their life in a way that they choose.
Responsibilities
* Conduct holistic, face-to-face Care Act Assessments, reviews and support planning using a strengths‑based approach to enable independence and choice.
* Work across Lancashire as part of the Urgent Care Acute and Prisons Service, supporting hospital discharges and preventing unnecessary hospital admissions.
* Utilise the Discharge to Assess model to ensure assessments are performed away from the hospital environment.
* Collaborate with the wider multi-disciplinary team to coordinate community intermediate care services, including reablement and residential rehabilitation, seven days a week.
Qualifications and Requirements
* Qualified Social Worker with current registration and relevant experience in adult social care and Care Act assessments.
* Empathy, drive to make a difference, and the ability to find creative ways to help people live as independently as possible in a place they call home.
