Overview
We are currently seeking compassionate and dedicated Support Workers with experience in mental health support to join a person-centred service focused on empowering individuals to live fulfilling and independent lives within the community.
Responsibilities
* Provide day-to-day support to individuals with mental health needs in accordance with their personalised care plans.
* Assist with daily living activities such as meal preparation, medication prompts, budgeting, and personal care when required.
* Support individuals to access community services, social activities, education, and employment opportunities.
* Build positive and trusting relationships with service users, encouraging independence and confidence.
* Monitor and report changes in mental health, behaviour, or support needs, ensuring appropriate action is taken.
* Maintain accurate and timely records of support provided, incidents, and progress notes.
* Work collaboratively with families, health professionals, and other stakeholders to provide consistent, high-quality care.
* Promote safeguarding and uphold the rights and dignity of all individuals in your care.
* Participate in regular team meetings, training sessions, and continuous professional development.
