Main duties
We are seeking compassionate and dedicated nurses to join our dynamic team, delivering care directly in patients’ homes and residential facilities within the community. You will assess and develop management plans for complex and acutely unwell patients, guiding unscheduled nursing care to ensure timely and effective support for priority patients.

Person Specification
Qualifications
* Essential: Registered Nurse with current NMC registration
* Nursing degree or equivalent
* Substantial years of post-qualification experience
Desirable
* V300 prescribing non-medical prescriber
* Non-Medical Prescribing qualification (V150)
* Community Nursing Specialist Practitioner qualification
* Community experience
Knowledge and Skills
* Demonstrate evidence-based clinical knowledge
* Effective communication skills
* IT skills
* Proactive problem solving
* Ability to travel across the Trust footprint as required
