JobDescription:
To provide ahighquality clinical pharmacy service as part of themultidisciplinary team to medical specialties patients to ensuremaximum efficacy safety and economic drug use and to demonstrateprofessional accountability to enhance patient care. Theresponsibilities will include:
* Regularattendance of consultant ward rounds and multidisciplinary teammeetings.
* Demonstration of expert clinicalknowledge
* To make clinical decisions wherelimited/conflicting information isavailable.
* To ensure compliance with localpolicies including medicines policy formulary and microbiologyguidelines. To challenge prescribing practices wherenecessary.
* Promotion of high quality andcosteffective prescribing.
* To be the mainpharmacy contact for a designated area in Medicine or Medicalspecialties including acute medicine rheumatology respiratorydermatology or immunology.
* Development andreview of Patient Group Directions
* Developmentand review of Shared Careguidelines
* Implementation of relevant local andnational guidelines within the specialistarea
* Assist in the implementation andevaluation of Drug Usage Reviews and clinical audits and implementchanges in prescribing practice accordingly.
* Toliaise and work with other specialist pharmacists to share the bestpractice to strengthen patient focused pharmaceuticalcare.
* To assist in the planning and organizingof a clinical pharmacy service to acute medicine and medicalspecialties to patients in our service in both the inpatient andoutpatient settings.
* To qualify as anindependent prescriber and where appropriate to improve patientcare and safety to continue to develop the role of pharmacistindependent prescriber within a designated clinicalarea.
* To provide clinical screening anddispensing support to the Medicine pharmacy team and medicalsatellite to ensure the accurate and timely dispensing ofprescriptions. To assist with the timely screening and checking ofdischarge prescriptions.
* To provide medicinesmanagement reviews for complex patients.
* Toliaise with doctors administration staff and patients to ensure thesafe and smooth running of the home deliveryservice.
* To deal effectively with situationswhere medication errors have occurred including liaising directlywith patients and staff concerned.
* Assist inresponding to complaints from patients and the public in relationto medicines management issues asrequired.
