Qualifications:
* Bachelor’s degree in healthcare preferred.
* Licensed Practical Nurse, Registered Nurse, or Social Worker
* CPR certification required.
Previous case management experience in a hospital, long-term care, or Managed Care setting preferred.
* Knowledge of growth and development principles across the lifespan.
* Effective time and resource management skills.
* Experience with behavioral and psychological aspects of care.
* Advanced computer skills.
* Strong communication skills to promote effective interaction at all levels.

Essential Duties and Responsibilities:
* Coordinate care management functions with hospital departments, external organizations, and healthcare facilities.
* Act as a patient advocate, investigating and reporting adverse events.
* Assist in staff education on resource utilization, discharge planning, and psychosocial care.
* Promote efficient utilization of clinical resources.
* Support physicians in discharge planning, problem resolution, and achieving desired patient outcomes.
* Communicate regularly with physicians and participate in rounds to address issues.
* Coordinate and facilitate patient, family, and caregiver education.
* Accurately document assessments, interventions, and outcomes per standards.
* Engage in ongoing educational activities.
* Maintain HIPAA compliance.
* Participate in quality improvement initiatives.
* Follow safety and infection control policies.
* Use interpersonal skills to facilitate teamwork and communication.
* Perform other case management tasks as assigned.
Supervisory Responsibilities: Not specified.
Language Skills: Proficient in English, both verbal and written.
Physical Demands: Long periods of sitting, bending, stooping, manual dexterity, responding quickly to emergencies.
