Stroke Specialist Nurse Practitioner

An exciting 12-month secondment opportunity has arisen within ourStroke Service to work as a Band 7 Stroke Specialist Nurse Practitioner,playing a pivotal role in ensuring the delivery of high quality and safe careto our patients throughout the stroke pathway, from the emergency department tothe ward, including supporting the delivery of the 24/7 thrombolysis service at South Tyneside and Sunderland NHS Foundation Trust.
We are seeking a knowledgeable individual with highly developedinterpersonal and communication skills and has a passion for deliveringexcellence in care, driving improvements and developments across the strokeservice.
Successful applicants will need to demonstrate an interest andsound clinical knowledge as well as an understanding of the specialist care andmanagement required by patients across the pathway which includes inpatient,outpatient, and an outreach service to support patients, carers and staff withspecialist advice, education and training in Stroke and its associatedcomplications. Main duties of the job
Main Duties of the post holder will include:
Clinical
Ensure provision of expert specialist advice andknowledge to patients, carers, and staff regarding stroke management
Accountable for the planning and delivery ofpatient care in relation to stroke.
Pro-actively manage own workload and clinicalcaseload of patients, making decisions regarding care, treatment and supportrequired.
Liaise with Stroke Physician Consultants,co-ordinate the delivery of the stroke patient pathways using effectiveevidence-based practice and national standards for patient safety andmonitoring.
Manage/lead specialist nurse led clinics andplaying a key role in optimising complex treatment regimes, supporting hospitaldischarge and preventing avoidable hospital admissions.
Managerial
Attend meetings regarding stroke care andrepresent STSFT as an organisation.
Participate in service meetings, stroke CG anddirectorate level discussions.
Contribute to MDT meetings ensuring patients areappropriately reviewed and treatment options discussed.
To represent the service and play a key rolein ensuring that national and local standards/targets are met.
Audit /Research / Governance
Participate in audit, research and governance activityrelevant to the stroke service.
Continuously improve clinical care standards inline with the STSFT CG framework

To coordinate the delivery and development of the specialist service through the useof effective evidence based practice in line with agreed national standards for patientsafety and monitoring.
To contribute to the organisation of multidisciplinary, patient centred care.
To be accountable for the planning and delivery of safe, effective and compassionatepatient care in relation to dedicated pathways of care, working as an autonomouspractitioner within the specialism.
To provide specialist clinical advice and support to patients and clinical staff withinthe Trust on the assessment, management and care of patients within the specialistarea.
To continuously improve clinical care standards in line with the clinical governanceframework.
To exercise accountability as set out in the NMC Code.
To ensure effective communication internally and externally regarding patient careand resources.
Provide education and training to clinical staff across the Trust.
Provide expert specialist clinical advice and support to patients and clinical staff withinthe Trust on the management and care of patients within the specialist area.
To provide direct access for patients, either face to face or telephone consultation.
To represent the service nationwide.
To play a key role in ensuring that national and local standards/targets for specialistarea are met.
To manage/lead practitioner led clinics in relation to reviewing patients and play a keyrole in optimising complex treatment regimes, facilitating hospital discharge andpreventing avoidable hospital admissions.
To undertake research relevant to the needs of the specialist area.

Person Specification
Qualifications
* Registered Nurse (adult).
* Degree in nursing studies or healthcare related subject (UK equivalent).
* Masters level study, or equivalent specialist study / experience.
* Mentorship certificate / Practice Assessor training or demonstrable equivalent experience.
* Clinical skills accreditation.
* Non-medical prescribing or willingness to complete.
* Leadership and Management course.
* Qualification in specialty.
Experience
* Extensive post registration experience, some of which will be at a senior level within the required specialist area of practice.
* Experience of care delivery.
* Experience of the specialism.
* Experience of leading a team/supporting junior staff.
* Evidence of innovation in practice.
* Evidence of change management.
* Evidence of practice development.
* Experience of coordinating care for a caseload of patients.
Physical Skills
* Ability to assist patients to move as per Trust Moving and Handling Policy.
* Able to travel across sites.
Skills & Knowledge
* Awareness of national health and social care policy.
* Knowledge of best practice in area of specialism.
* Thorough understanding of expanded roles / professional practice issues.
* Clinical teaching experience.
* Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassurance skills are required.
* Able to work without supervision.
* Competent in the use of IT packages.
* Evidence of competency in relation to clinical practices appropriate to the post.
