Deputy Community Nurse Team Lead – Arbennek Probus

Join to apply for the Deputy Community Nurse Team Lead – Arbennek Probus role at Phoenix MCT. Band 6. Main area: Adult Community Services. Contract: 12 months (Fixed term contract or secondment considered. Fixed term contract ends 31/01/2027). Hours: Part time – 22.5 hours per week (to include evening and weekend working). Job ref: 201-25-779. Site: Unit 5, Chenoweth Business Park. Town: Truro. Salary: £38,682 – £46,580 Pro Rata. Salary period: Yearly. Closing: 05/11/2025 23:59.

Overview and context

This role is within the Primary Care Network, providing holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes. The post holder will work with the team to ensure delivery of person-centred care, and will lead the team in a flexible, evolving environment that supports integrated working.


Main duties of the job

* As an autonomous practitioner, manage a clinical caseload within a community nursing team with guidance from the Community Nurse Team Leads, Community Matrons and Integrated Care Team Leads.
* Provide holistic nursing assessment, physical examination, history taking, and treatment planning to prevent hospital admission and support timely discharge from hospitals and other care settings.
* Develop and implement personalised care plans in partnership with patients, carers and multidisciplinary teams to promote independence and quality of life.
* Lead and provide clinical management and leadership to all staff within the team, ensuring safe, effective and evidence-based care.

Person specification

Education, Qualifications and Relevant Experience (Essential)

* Current professional registration NMC
* Experience of working with patients with complex and challenging needs
* Post graduate specialist training to degree level in relevant subject area

Skills and Aptitude (Essential)

* Excellent written and verbal communication skills
* Strong commitment to teaching and learning
* Budget management skills

Knowledge and Abilities (Essential)

* Knowledge and application of workforce planning, supervision and appraisals
* Strong team player, capable of motivating and engaging colleagues
