Job Summary

We have an exciting opportunity for a rehabilitation-focused Senior Occupational Therapist to join our Inpatient Rehabilitation Service on Welney Ward, Princess of Wales Hospital, Ely. This position works with patients on our inpatient wards to support patients out of hospital and offer holistic clinical assessment, rehabilitation, discharge planning and specialist therapy knowledge.

Successful applicants will play a vital role in supporting patient flow through the pathways as well as providing clinical skills and knowledge to the MDT. This position offers flexibility, variety and the opportunity to be involved in the transformation of our inpatient rehabilitation service. You will be supported by a band 7 clinical lead. Key Responsibilities

* Occupy an autonomous practitioner role within the integrated multi-professional team.
* Be responsible for own caseload and undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
* Identify patient needs, agree goals and provide appropriate therapy interventions, provide adaptive equipment, rehabilitation programmes, and make recommendations for minor adaptations in line with current legislation.
* Keep the patient at the centre of care, ensuring patient and carer participation in decision making. Provide support and education to peers, new staff, non-registered staff and students.
* Be accountable for a delegated caseload held by Band 5 Therapists and Assistant Practitioners.
* Prioritise all patients’ tasks according to clinical need and allocate or signpost appropriately. Provide a high standard of care.
* Work in various community settings within CPFT as required, completing access and home visits.
* Be professionally and legally accountable for all aspects of your own and delegated work.
* Work within codes of practice and professional guidelines.
* Use evidence-based practice to ensure clinical care is effective and appropriate.
* Promote independence and avoid acute hospital admission where possible. Organise and manage own time, delegating work appropriately and conduct risk and health and safety assessments, reporting changes immediately.
* Demonstrate the importance of gaining patient and carer consent in all interventions.
* Communicate verbally with patients, carers and colleagues using tact and persuasive skills, adapting where patients have communication difficulties.
* Actively manage positive relationships with other agencies involved in the patient’s pathway and be involved in the development of the service and new model of care.
* Attend MDT meetings, case reviews and family meetings to liaise appropriately with members of the MDT and other agencies.
* Maintain accurate and timely written records in compliance with Trust policy and HCPC guidance, reporting any issues as appropriate.
* Provide specialist advice to all members of the multi-disciplinary team, service users and others regarding changes to care plans and patient progress.
* Work within Trust and COT guidelines and maintain a good working knowledge of national and local standards of practice in line with legal and operational requirements.
* Demonstrate a good understanding of social services and health legislation.
* Support day-to-day planning and delivery of high quality care.


Key Requirements


Education / Qualifications

* Degree or equivalent (e.g., BSc, MSc or Diploma) entry-level qualification into the OT profession.
* Current HCPC registration.
* Documented evidence of continuing professional development.
* Willingness to undertake Fieldwork/Practice Educator qualification.
* Member of COT (desirable).
* Fieldwork/Practice Educator qualification (desirable).


Experience

* Relevant postgraduate clinical experience in a variety of settings.
* Clinical supervisor experience.
* Experience and knowledge of minor adaptations and provision of equipment.
* Community experience (desirable).
* Experience and knowledge of housing legislation and major adaptations (desirable).


Skills & Abilities

* Commitment to high-quality care and service provision.

* Excellent communication and report-writing skills.
* Ability to assess, plan and implement care.
* Support for Integrated Care Workers.
* Standard keyboard skills and IT proficiency (Word).

Physical Requirements

* Kneel, bend & stoop and work in cramped environments.
* Move and manoeuvre people and equipment.
* Manoeuvre patients using moving and handling aids.
* Travel around work base locality.
