As a Care Coordinator, you will help ensure residents receive high-quality, person-centred care and work with the care team to manage residents’ needs, preferences and goals.
Responsibilities
* Coordinate and manage care for residents, considering individual needs, preferences and goals.
* Organise shifts, administer and record medication; liaise with health professionals.
* Record information in residents’ computerised care plans.
* Assist with induction, training and supervision of support staff.
Qualifications
* Experience supervising a care team.
* Empathy and understanding of residents’ needs.
* Good leadership, communication and teamwork skills.
* Attention to detail; well-organised and methodical.
* Flexibility around working schedules, roles and responsibilities.
* You will have, or be working towards, NVQ3 in Health & Social Care.
