Job Title: Qualified Social Worker – Adult’s Neighbourhood Care Management Team
Location: Leeds
Rate: £35ph
An exciting job role has recently become available in Leeds. We are looking for a new member for their Adult’s Neighbourhood Care Management Team.
What will your responsibilities be?
You will ensure all aspects of a patient’s care are organized and communicated across providers. You will conduct regular evaluations of the patient’s health status and develop personalized care plans to meet the specific needs of each patient. You will provide access to social services, mental health support, and community resources. Additionally, you will take personal responsibility for continuing professional development and maintaining performance levels. You will manage your time effectively and respond flexibly to service needs. You will provide support and cover for the Team Manager and/or Principal Manager. You will actively participate in the performance management system, including supervision, PAR, team meetings, and service reviews. You will follow departmental health and safety procedures. You will develop skills through work experience in various divisional situations, such as secondments, job rotation, and shadowing. You will contribute to information management processes. You will support social work students on placement under the direction of the Principal or Practice Manager, acting as a workplace supervisor or Practice Teacher if appropriately trained. You will need to understand and integrate into practice the key policies of the council in relation to social care with adults, especially multi-disciplinary working, integration, and whole family approaches.

Qualifications and Experiences:
You must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
