The Oxfordshire Health and Homelessness Inclusion Team (OHHIT) is an integrated partnership operating across housing, health, care and third sector systems.Formed in 2021 as part of the Department for Health and Social Care's Shared outcomes initiative, our team was one of 17 sites nationally looking to test innovative ways to work across systems, with the core aims of:
Supporting planned, safe discharges from hospital for people experiencing or at risk of homelessness - avoiding discharges to street.Increase access to mainstream services in the community - avoid unnecessary (re)admissions to hospital and reduce inequalities.Prevent rough sleeping and homelessness.
Main duties of the job
* Carrying out holistic occupational therapy assessments and interventions with individuals experiencing or at risk of homelessness across Oxfordshire (predominantly Oxford city). This may include short or long term work, and a range of physical and mental health needs
* Working as part of an MDT to enable people to overcome barriers to accessing mainstream health and social care services, where appropriate.
* As part of the wider clinical team, ensure that the care and support needs of individuals within Step Service accommodation are assessed - in line with principles of Discharge to Assess (D2A)
* Contributing to ongoing development of the occupational therapy service within OHHI, and wider service delivery and process.
* Providing guidance or consultation to staff across health, social care, and third sector services to support understanding of occupational needs and therapeutic approaches.

Qualifications
* Current professional registration: Registered Mental Health Nurse, Occupational Therapist
* Post graduate training relating to client group
Experience
* At least 2 years post qualification experience.
* Experience of working with the homeless community.
Knowledge
* Specialist knowledge and clinical expertise in community mental health in line with professional requirements
* Demonstrate an understanding of clinical risk assessment and management
* Have a developed understanding of Clinical Governance
