We have a fantastic opportunity for a qualified social worker with North Somerset Council, working within adult services on risk and care needs assessments. The post holder will be responsible for a caseload of varying complexity receiving and responding to referrals from various sources to co-ordinate hospital discharge, record continuing health care needs and identify and record unmet need.
This role is advertised as £33 p/h
Key roles include but are not limited to:
1. Assessing and coordinating health and social care needs
2. Financial assessments
3. Developing, implementing and monitoring care plans
4. Ensuring all service user's, especially those with disabilities, have equal access to community services
The minimum qualifications for this role are:
5. Social Work qualification
6. Social Work England registration
7. Post-qualification work experience is desirable
