Health Care Support Worker - Band 3 - Ward 4, Harplands Hospital. There is an exciting opportunity to work as a Health Care Support Worker Band 3 within Ward 4, Harplands Hospital. We are searching for dedicated Health Care Support Workers who have experience, passion and pride when caring for patients. The Trust offers flexible working (early, late, night and twilight shifts).
The post is based at Harplands Hospital. We offer free parking to all staff and support career development, including Training Nurse Associate and Nurse Apprentice schemes. There is also an opportunity to obtain your Mental Health Certificate. NSCHT delivers a shared care service on Ward 4 which aims to provide high-quality health & social care that promotes social inclusion & independence for people with mental and physical health problems. The team is multi-disciplinary, comprising nurses, medical staff, a physiotherapist and an occupational therapist.
Details: Band 3, Agenda for Change; Salary £24,937 to £26,598 per year; Permanent, Full-time; Location: Harplands Hospital, Hilton Road, Stoke on Trent, ST4 6TH.
Job responsibilities
* Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
* Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team.
* Relates appropriately with service-users, carers, colleagues and others at all times, maintaining professional boundaries.
* Ensures that actions support the equality, diversity, rights and responsibilities of individuals.
* Identifies, minimises and reports risks of danger, harm and abuse to senior colleagues.
* Minimises risks to self and others by undertaking safe working practices.
* Works within agreed and established Trust policy and operational procedures at all times.
* Works with other Trust services to promote seamless service-user person-centred care from referral/admission to transfer and discharge on identified care pathways.
* Contributes to planning and delivery of planned therapeutic activities with an identified group of service users.
Person specification
Qualifications
* Advanced Apprenticeship in Mental Health and Social Care or NVQ 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or Functional Skills).
Experience
* Experience of working with people with Mental Health Needs as a member of a multi-disciplinary team.
