Job Summary
We are seeking a compassionate and experienced Care Manager to coordinate and oversee comprehensive care plans for clients with behavioral health needs. The ideal candidate will have a strong background in social work, crisis intervention, and addiction counseling, with the ability to manage complex cases involving child welfare, Medicare, Section 8, and workforce development. This role requires excellent communication skills, crisis management expertise, and a dedication to improving client outcomes through personalized support and mentoring.
Duties
* Develop and implement individualized care plans tailored to clients' behavioral health, child welfare, and social service needs.
* Conduct crisis intervention and manage urgent situations effectively to ensure client safety.
* Provide addiction counseling and motivational interviewing to support recovery efforts.
* Coordinate services related to Medicare, Section 8 housing, and workforce development programs.
* Supervise and mentor junior staff or interns, fostering a collaborative team environment.
* Collaborate with community agencies, healthcare providers, and families to facilitate comprehensive support systems.
* Monitor client progress through regular follow-ups and adjust care plans as necessary.
* Assist clients in navigating social services systems, including child welfare and housing assistance programs.
* Maintain accurate documentation of case activities while ensuring compliance with all regulatory standards.
Requirements
* Proven experience in behavioral health, social work, or case management roles.
* Strong crisis management skills with the ability to handle high-pressure situations calmly.
* Expertise in addiction counseling, motivational interviewing, and crisis intervention techniques.
* Supervisory or mentoring experience preferred.
* Knowledge of child welfare laws, Medicare regulations, Section 8 housing policies, and workforce development initiatives.
* Excellent communication and interpersonal skills to engage clients effectively.
* Ability to work independently while collaborating within a multidisciplinary team.
* Compassionate approach with a commitment to client-centered care and community service excellence.
* Relevant certifications or licenses related to social work or behavioral health are highly desirable.
