Social worker / Nurse / Occupational Therapist - MH Practitioner
You will deliver a high quality comprehensive mental health service within the Early intervention for Psychosis team. Insight is a multi-disciplinary team that has been undergoing development and an exciting opportunity to be part of this growth has come up. Insight provides well rounded care following the Early Intervention for Psychosis standards. We offer those experiencing first episode psychosis a 3 year service and within the 3 years we aspire to offer a well rounded goals based care approach with the involvement of our clients throughout the journey. Insight offers the following services delivered in house, set out by the standards of Early intervention. There will be training opportunities for those interested in these areas.

* Carers Group
* Coffee Group
* Acceptance Commitment Therapy
* Psychoeducation Group for friends family & carers
* Care coordination working with outside agencies/teams to ensure effective well rounded care is delivered
* Peer support is part of the model and we are working hard to develop this area

Main duties of the job
The role involves managing a defined caseload of 15 with a focus on providing expert assessment and treatment for people with complex mental health difficulties, primary diagnosis first episode psychosis and also working with their carers who support this client group. This will include recovery co-ordination (design & co-ordination of treatment packages).
You will contribute to the clinical leadership of the team, providing supervision for other members and participating in team/service development and evaluation. Communication is key; working relationships, being open to feedback and giving feedback in a productive and open minded approach. Planning/organisational skills. Responsibility for Patient/Client Care, Treatment & Therapy. Responsibility for Policy/Service Development Implementation. Responsibility for Finance/Equipment/Other Resources. Responsibility for Human Resources e.g. Supervision/Training/HR Advice/Management. Responsibility for Info Resources/Administrative Duties. You will need to travel to engage with people in their own residence or hospital settings whilst planning discharge. They may need to sit for prolonged periods to compile complex reports.

JOB PURPOSE

Duties and responsibilities
Communication and Working Relationship Skills
* Contribute to the team’s engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies.
* Impart highly complex and sensitive information to patients and families/carers re assessment, care planning, treatment and review
* Work with multi-disciplinary teams to ensure the provision of consistent well-coordinated care
* Coordinate with external agencies involved in care (voluntary and private sector, police, social services, housing, employment, etc.)
* Engage in service user/carer groups; liaise with a wide range of professionals and agencies
* Support safe and effective day-to-day operations of the service including handling clinical enquiries
* Provide and receive information some of which may be contentious, sensitive and highly complex to and from individuals and their families/carers
* Provide high standards of written and verbal communication in line with professional documentation standards
* Participate in and chair clinical meetings, including CPA and non-CPA reviews, professional meetings, and clinical risk meetings

* Undertake complex risk assessments and develop risk management plans

Planning and Organisational Skills
* Organise and facilitate complex meetings including CPA and non-CPA professional and risk meetings

* Plan and organise individual packages of care and ensure reviews are held within required timeframes (e.g., Section 117, Funding Panels, CPA)

Responsibility for Patient/Client Care, Treatment and Therapy
* Ensure caseload is well managed and care/treatment aligns with practice standards and recovery coordination
