Social Care Team Lead – Urgent Community Response

We are seeking a motivated, confident and compassionate Social Care Team Lead to join our integrated multi-professional team. This role is central to ensuring safe, timely, and effective care for people in crisis at home across East Sussex – helping prevent avoidable hospital admissions and supporting people to remain where they feel most comfortable.
Main duties of the job
* Provide daily operational support to Team Leads and shift coordinators
* Oversee effective rostering and workforce deployment
* Lead and mentor staff through supervision, live observation and feedback
* Support training, education and competency assessment frameworks
* Drive continuous improvement, service development and project work
* Support complex case management and inter-professional working
* Ensure safe, high-quality care aligned with Trust policy and safeguarding standards
* Foster a positive, inclusive and collaborative team culture
About you
You’ll be an experienced and motivated leader with a strong grounding in adult health or social care, bringing both confidence and compassion to your work.
You will have:
* A good general level of education, including English and Maths, and be educated to degree level in a relevant field or have equivalent knowledge and experience gained in practice.
* Leadership training or qualifications at Level 4 or Level 5.
You will bring:
* At least 3 years’ experience working with adults in health or social care
* Experience supervising or managing staff, including performance management
* Strong communication skills and confidence in supporting complex situations
* Ability to lead calmly under pressure and navigate challenging conversations
* Commitment to high-quality care, learning culture, and continuous improvement

Qualifications
* Good general level of education including English and Maths
* Educated to Degree level in a relevant field or equivalent level of knowledge and experience
* Leadership qualification/training at Apprenticeship Level 4 or 5 (e.g. Management, or Business and Administration) or equivalent level of knowledge and experience
* Other recognised management or leadership qualification
* Evidence of relevant post graduate CPD.
Experience
* Minimum of 3 years’ relevant experience of working directly with clients and carers within Adult Health and/or Social Care Services
* Experience in a Management or Operational role within Adult Health and/or Social Care Services
* Experience of risk assessment and management in the home environment
* Experience of doing complex assessments and making sound judgments
* Experience of inter-professional working
* Experience of change management within a service in response to research, policy, guidance or legislation
* Experience of undertaking audit and identifying and implementing changes required
* Experience of managing staff, including management of performance, and their education and training development
* Experience of supervision and appraising staff
* Delivering quality services along evidence-informed guidelines
* Experience of designing and delivering training
* Skilled in the use of reflective practice
* Evidence of planning and policy development
* Experience of Service Development
* Experience within acute and community settings
* Experience of preparing / presenting service developments at a senior level
Skills
* Experience of supporting and assessing staff with completion of the Care Certificate

* Understanding of the future direction of the NHS
* Understanding of the unique demographic within East Sussex and the impact of service demand
* Experience of inputting into business cases
* Advanced IT skills

Salary
£39,959 to £48,117 a year, pro rata
