We are seeking an enthusiastic individual to take on the role of Practice Nurse. The role is to ensure the delivery of an effective Practice Nurse service to the practice population, delivering care appropriately according to practice policy, protocols, guidelines, and procedures.
Main duties of the job
* To carry out a wide range of treatment room duties, e.g., venepuncture, new registration checks, blood pressure checks, ECGs, vaccinations, and immunisations.
* To run Chronic Disease Clinics such as CHD, Diabetes, and Respiratory.
* Offer appropriate lifestyle advice to patients.
* To provide support to the nursing team to ensure the smooth running of the nursing service.

Job responsibilities
Clinical Practice:
* Assess, plan, provide, and evaluate treatment programmes including, but not limited to:
* Manage and treat patients with long-term conditions in line with national/local policies and practice needs, including Home Visits.
* Review medication for therapeutic effectiveness in accordance with evidence-based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments.
* Support and manage health needs of women presenting for Family Planning and Cervical Cytology Consultations.
* Implement and participate in Vaccination and Immunisation Programmes for both Adults and Children.

Person Specification
Qualifications
* Qualified Nurse.
* Experience of GP Practice Nursing.
* Full UK Driving Licence.
* Prescriber qualification.
* Diploma in respiratory.
