Job summary
To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practice's multidisciplinary team. The care navigator will also undertake reception duties, supporting the administrative team in delivering a polite and professional service to the entitled patient population.
Main duties of the job
The core responsibilities of the Care Navigator include, but are not limited to:
1. Processing and signposting patients to the appropriate healthcare professional based on their presenting condition.
2. Answering incoming phone calls, transferring calls, and handling requests appropriately.
3. Processing patient requests for appointments and repeat prescriptions.
4. Initiating contact with and responding to requests from patients, team members, and external agencies.
5. Entering read-code data on SystmOne and performing data entry for new and temporary registrations.
6. Photocopying documentation as needed and managing relevant patient information.
7. Directing information requests, such as SARs, insurance/solicitors letters, and DVLA forms, to the administrative team.
8. Managing queries efficiently and carrying out system searches as requested.
9. Maintaining a clean, tidy, and effective working area, including monitoring the reception area and noticeboards.
10. Supporting clinical staff with general tasks and actioning incoming emails.
