We are looking for a dynamic experienced Practice Nurse to take on the role of Lead nurse in this community based village surgery. We are a friendly, supportive team of 4 Partners, 4 salaried GP's, 1 Practice Nurse and 3 Health Care Assistants. The clinical team meet daily for coffee and monthly clinical meetings are supported by an experienced admin team.
Main duties of the job
You will be responsible for the delivery of safe, effective and evidence based nursing care. You will be expected to work collaboratively with the GP Partners and the Practice Manager, with your main responsibility being the provision of clear leadership to the nursing team. You are to ensure a high quality patient centred service, whilst contributing to service development, clinical governance and quality improvement.
You will be expected to lead the nurse clinics on diabetes ( and asthma and COPD if keen). You will provide immunisations, vaccinations, cervical cytology and wound care, along with health promotion and wellbeing for those with /or at risk of long-term conditions. You will lead the team in planning and implementing local guidance, protocols and standards, whilst leading on QOF and the delivery of LTC management.
About us
You will be supported by an experience Practice Nurse, along with three very competent Health Care Assistants. The team works very well together, ensuring that our patients get the best care possible. All have developed good relationships with the patients, many of whom have been with the surgery for a long time.
The Nursing team work collaboratively with the GP's, attending and taking an active part in clinical meetings. They are supported by an experienced administrative team. We aim to provide as much continuity of care, enabling patients to build up a relationship with the team as much as possible.
The hours are flexible - we are looking for a minimum of 27 hours a week
Job responsibilities
Clinical:
Lead the nurse led clinics on diabetes, (and asthma,COPD if keen)
Provide immunisations, vaccinations, cervicalcytology, wound care,
Responsible for the infection control protocols
Meet the patients needs presenting for woundcare
Health promotion and wellbeing plus individualplans for patients with/at risk of a long-term condition
Leadership/Managerial:
Be the clinical lead in the delivery of Practicenursing services to patients, ensuring that the needs of the patient are thepriority
Lead the nursing team in the planning,implementing the local guidance, protocols and standards.
Lead on QOF and the delivery for LTC management,maximising income
Attend and participate in Practical Clinical andEducational meetings as required
Delivery of a quality service
Plan and manage the nurse appointments and rotas
Interpret national strategies and policies inlocal implementation strategies that are aligned to the values and culture ofgeneral practice
Monitor the performance of the nursing team
Prioritise, organise, and manage own and othersworkload in a way that maintains and promotes quality
Lead on infection control within the practice
Professional:
Maintain NMC registration and CPD
Adhere to all mandatory training requirements
Person Specification
Qualifications
* 1st Level Registered Nurse (RGN/RN).
* Current Nursing and Midwifery Council (NMC) PIN.
* Enhanced DBS clearance.
* Evidence of ongoing professional development.
* Specialist diplomas/modules in chronic diseases (Diabetes, Asthma, COPD).
* Experience in developing protocols and clinical guidelines.
Experience
* Minimum 5 years post-registration experience.
* At least 2 years recent experience in a primary care/general practice setting.
* Advanced clinical skills: Cervical cytology, immunisation and vaccination, wound management, and minor illness assessment.
* Strong leadership, team management, and mentoring capabilities.
* Ability to lead on infection control and clinical audit.
* Ability to manage and prioritise workload under pressure.
* Confident, polite, and empathetic communicator.
* Forward-thinking with ability to implement change and improve services.
* High levels of integrity and a commitment to team effort.
* Strong IT skills including SystmOne
* Experience in chronic disease management (e.g., Asthma, COPD, Diabetes).
* Knowledge of Patient Group Directions (PGDs) and clinical governance.
* Understanding of health promotion strategies and public health issues.
* Experience in protocol / SOP writing
