Summary
We are seeking a dedicated Discharge Facilitator to join our team at NHS Professionals to support our team at Royal Berkshire Hospital. The post holder will work as a Junior discharge facilitator for the Rapid Community Discharge Team based at RBH. The post holder will work closely with and be supported by the senior team members. The postholder will need to work closely with key stakeholders within the Acute Trust – delivering a community driven, discharge to assess approach to discharge – enabling the effective flow of patients into the right service at the right time.
Responsibilities
1. The post holder will be supported by senior staff on a shift to ensure the effective discharge of patients on pathways 1,2 and 3, for the residents of Berkshire West according to the relevant government Hospital discharge requirements.
2. To identify and work with senior colleagues to identify any obstacles to discharge and work to resolve, these escalating as appropriate to progress their discharge, whilst recognising the need for patient safety.
3. The facilitator will be supported to adopt a problem- solving approach negotiating and facilitating effective discharge in circumstances where there may be differing view of a patient needs.
4. The Junior Facilitator will work with senior colleagues and have a collective responsibility for the development of improved hospital discharge flow - working with a QI approach and taking responsibility on shift for the timely and accurate provision of performance reports.
5. The post holder needs excellent communication skills dealing with CCGs, acute Trust's and social services colleagues. They need to be able to communicate difficult information in a sensitive manner.
6. The post holder will lead discharge calls with services, which will include the planning and onward journey of the patient.
Qualifications
* Significant experience of working in a community services setting.
* Previous experience of multi-agency collaboration working across professional boundaries.
* Excellent organizational skills and attention to detail.
* Knowledge of hospital discharge process.
* Strong microsoft and IT skills
* NHS experience is preferred.
