Responsibilities
The Cumberlege Intermediate Care Centre (CICC) is a 22‑bed intermediate care unit that provides stroke rehabilitation. The role requires skilled and experienced staff to assess and treat patients with stroke/neurological diagnoses, working as part of a large and dynamic multidisciplinary team.
* Physiotherapy assessment, treatment and evaluation of stroke/neurological and frailty patients in a community rehabilitation setting.
* Participate in multidisciplinary rehabilitation services within the Intermediate Care Service, collaborating with Rehabilitation Assistants, Therapy Assistants, Physiotherapists, Occupational Therapists, Speech and Language Therapists and Nursing colleagues.
* Work autonomously to provide evidence‑based, high‑quality patient care that is personalised to each patient’s goals and needs.
* Deliver a compassionate, dignified and respectful service to patients, ensuring adherence to NHS constitution values and raising concerns with the relevant line manager through the appropriate Trust processes.
* Collaborate with colleagues across the Intermediate Care Service to optimise patient outcomes.
