You'll support individuals who frequently access services-empowering them to take control, avoid crisis, and reconnect with their communities. We welcome applications for both full-time and part-time. Depending on your location, you may cover East Riding of Yorkshire and North Lincolnshire, or focus on one area although flexibility would be required for cover over both areas based on service demand. What does a typical day as an Outreach Caseworker involve? Supporting individuals with complex needs to access the right help and avoid crisis. Identifying social factors driving high use of emergency services. Building trust with people facing challenges like trauma, addiction, and mental health issues. Reducing stigma and shifting behaviours through community engagement. Working closely with partners like A&E, GPs, housing teams, and mental health services. What will you bring to the role of Outreach Caseworker? Experience supporting vulnerable people with complex needs, e.g. substance abuse, mental health conditions. Strong communication skills and the ability to build trust. Confidence in delivering casework, person-centred planning and risk assessment. Quick thinking and effective negotiation in fast-paced settings. Accurate record-keeping and attention to detail. Ready to make a difference?
