Overview
37 hours per week • 2 positions available. As a social worker in the Community Learning Disability team, you will provide a strength-based, outcome-focused enabling response to adults affected by disability, frailty, learning disabilities, mental health and dementia, requiring a timely social work response. Working with a multidisciplinary approach, your role will include assessment of needs and, where eligible, care and support plans, and where appropriate care and treatment plans, to promote the independence and wellbeing of individuals, their families and carers, focusing on 'what matters' to people. Where practicable, you will focus on prevention and self-management, enabling individuals to sustain independence through assessment, care and support and other practical interventions.
Responsibilities
* Provide a Social Work service with assessment of needs and, where eligible, development of care and support plans, and where appropriate care and treatment plans.
* Promote independence and wellbeing for individuals, their families and carers.
* Adopt a multidisciplinary approach to service delivery.
* Focus on prevention and self-management where possible.
* Ensure activities are aligned with what matters to people and consider practical interventions to support independence.
Requirements / Qualifications
* The ability to greet customers through the medium of Welsh is a requirement for this post.
* Protecting children, young people or adults at risk is a core responsibility of all council employees.
* Criminal records check: Enhanced with Adults & Children’s Barred List (DBS) is a requirement for this post.
