About the role:
1. Work intensively with patients for 12 weeks prior to and post-release
2. Design holistic resettlement plans to support the continuity of healthcare once our clients are released from prison
3. Complete secondary screening assessment of patients arriving to prison as part of Integrated Health and Wellbeing model of service delivery
4. Work closely with CGL teams, healthcare, prison partners and external support agencies to ensure effective coordination of release planning
5. Providing advocacy for access to partnership services
6. Contribute to risk management processes, including prison ACCT process as appropriate
7. Work closely with Through the Gate provision, Healthcare, Prison partners and External Support Agencies to ensure effective coordination of Release Planning
8. Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination
About you:
9. Have a strong understanding of substance misuse issues and experience of working within a similar field ideally within prison/criminal justice
10. Be an excellent communicator, both verbal and written with good IT skills
11. Proactive and able to work on own initiative with excellent time management and prioritising skills
12. Have a good working knowledge of mental health interventions, services, and good practice
13. Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
14. Ability to manage change successfully in a way that prioritises the needs of service users
15. Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
16. Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
