Band 7 £50,861 - £59,159 per annum
Plus Distant Islands Allowance of £1,406 per annum

We have an exciting opportunity for a Specialist Clinical Pharmacist to join an evolving Integrated Front Door team. We are seeking a motivated and enthusiastic pharmacist to lead on expansion of our Integrated Front Door service, encompassing Hospital at Home, Acute Assessment Unit, Urgent Care and Frailty, shaping the way rural healthcare is delivered within the remote-island location.
This post will cover patients within the Hospital at Home virtual ward as well as providing support for the Urgent Care and Acute Assessment teams. In addition, the post holder will be responsible for implementing and delivery polypharmacy reviews across the services and in particular working with the new Frailty team as part of the MDT performing Comprehensive Geriatric Assessments. There is a vision to expand the service in line with Scottish Government initiatives.
