An exciting opportunity has arisen for a motivated, compassionate and enthusiastic Band 5 Nurse to join the Learning Disability Intensive Support Team. You will be joining a highly supportive multidisciplinary team where regular supervision, open communication and professional development are prioritised - ensuring you feel valued, guided and confident in delivering best possible care. You will work across Hull and East Riding, supporting people with learning disabilities, their carers and family. There is a requirement for a full UK driving license and vehicle.

The successful candidate will work across the (IST) and the Learning Disability inpatient Unit ). This role provides the chance to develop skills and experience in two closely linked services whilst ensuring continuity of care for patients as they move through the Learning Disability Pathway. The role will also provide the opportunity to gain a wider understanding of the patient and family/carer journey.

As such we are looking for a flexible, forward think nurse who has the service user at the centre of everything they do.

Your experience will be vital in maximising a positive patient journey, primarily preventing admissions in the community however also, should the need arise, supporting the admission, treatment and subsequent discharge. Across both services we work as part of a multi-disciplinary team to provide a holistic approach in the care of those who access our service.


Main duties of the job

You will work to improve the health and well-being of individuals with learning disabilities by assessing needs, creating care plans, and supporting them to live independently.

You will work with individuals and their families to improve and maintain physical and mental health, overcome barriers to healthcare and social inclusion, provide advice, and support with daily life skills.

This role involves managing your own caseload, coordinating with other professionals, and ensuring the person is at the centre of everything you do.

Person Specification


Qualifications

* Degree/diploma in learning disability nursing (or in final year of Nursing degree programme)
* Have current NMC registration or student registration if in final year of training
* Willingness to undertake mentorship training
* Up to date training & registration as a mentor
* Evidence of relevant CPD - could include completion of short courses or post-graduate study.
* Completion of Nursing preceptorship


Knowledge

* Knowledge of the specific service area, team and role evidenced within the supporting information of application to vacancy.
* Experience of Community Nursing in prior employment or student placement.
* Understanding of caseload management responsibilities
* A broad understanding of relevant policy, legislation drivers and their application to Community Nursing
* Experience of managing a caseload of patients including triage and prioritisation of referrals and daily work
* Experience of mentoring pre-registration students
* Full understanding of relevant policy, drivers and their application to Community Nursing
* Full understanding/application of relevant clinical practice/standards/audit within Community Nursing


Skills

* Adequate IT skills required to utilise electronic patient records, electronic communications, clinical stock ordering system and Microsoft Office applications
* Effective inter-personal skills and experience for teaching, supervising and mentoring.
* Good time management skills and organisational skills to enable effective caseload management
* Ability to demonstrate ethical values and attitudes within a culture of equality and diversity
* Experience of use of SystmOne
* Ability to manage daily operations and maintain a safe working environment

£32,073 to £39,043 a yearEnhanced rate for weekend working if required
