Overview
The Wigan Specialist Community Mental Health Team (SCMHT) are looking to welcome an additional Community Mental Health Practitioner to the team. This post follows the first successful round of recruitment as part of our Community Transformation Plan and implementation of the streaming model of care in the SCMHT. The post is based at Claire House in Wigan with travel across the Borough to attend satellite clinics in Wigan and Leigh. The post is a full-time position (37.5 hours) with working hours 8am – 8pm, 7 days a week. Flexible and part-time working can be discussed.
Responsibilities
* Work with the depot team and run a regular clinic for service users to administer medication.
* Screen referrals and offer assessments to patients accepted to the service for assessment.
* Collaborate with patients and families to identify the appropriate care and treatment plan for their needs. This role would be non-caseloading.
* There are opportunities to support in a caseloading role within one of our streams to support an identified caseload of patients with severe and complex mental health conditions under the direct clinical supervision of a Senior Case Manager (Band 7).
* Manage duty calls from adult service users with severe and enduring mental illness, and from carers, friends and families, offering advice and support.
* Travel to satellite clinics and work across the SCMHT as service needs require.
Working arrangements and supervision
Staff wellbeing and development is paramount. Full supernumerary status will be provided to support induction to the team for the first 4 weeks in post. Your development opportunities will be supported as part of your induction process and reviewed regularly during line management supervision and an annual appraisal. Regular reflective practice sessions are encouraged.
Requirements and checks
All positions within the Trust are subject to satisfactory pre-employment checks. The Trust mandates that post holders who require a DBS subscribe to the DBS Update Service (enrolment is required for a fee of £16 per year and maintenance throughout employment).
