Patient Flow Coordinator

An exciting opportunity has arisen for the role of Patient Flow Co‑ordinator in the Clinical Site Team.
You will be required to cover the 7 day rota which will include days, evenings, weekends and bank holidays on a rotational basis. Cross site working will also be required.
The purpose of the Flow Co‑ordinator's role is to ensure that patients have a seamless journey and discharge and that the patient's pathway is effectively managed across site.
We are looking for a motivated and enthusiastic individual to join our dynamic team to contribute to high quality patient experience. You will play a pivotal role in supporting the Ward managers and Matrons to support patient pathways from admission to discharge/transfer.
We are looking for a caring and compassionate person who would like to make a difference to our complex patient group.
Main duties of the job
The flow coordinator is responsible for liaising with the Emergency Department, Clinical Site Team and other Flow Co‑ordinators and receiving areas to ensure effective communication regarding capacity is maintained and appropriate early escalation is facilitated.
Applicants should have recent experience in a health environment, be computer literate, have GCSE English or equivalent, possess good communication skills, and be able to work with multi‑professional teams. The applicant must be able to work flexibly, prioritise workload effectively, and use own initiative in certain work‑related issues.
Applicants will be required to work across both sites, Doncaster and Bassetlaw Sites.
Monday to Sunday – working hours 07:30‑20:00

Qualifications & Training
* Basic maths and literacy
* Vocational qualification at level 2 or equivalent relevant experience
* GCSE Grade C in Maths and English or educated to the equivalent
Knowledge & Experience
* Experience working with people using customer care skills
* Knowledge and understanding of current healthcare issues
* Experience of working in a healthcare setting
* Basic computer skills
