The Care Coordinator will be part of the Primary Care Network (PCN), responsible for managing the care of registered patients across practices within the PCN. A key part of this role involves working in the Care Homes Multi-Disciplinary Team (MDT) to improve care continuity by acting as a contact point for residents, families, and visiting professionals. This includes coordinating healthcare professionals, non-clinical staff, volunteers, and third-sector agencies involved in patient care.
Main Duties
The Care Coordinator will support MDT activities, including weekly ward/home rounds, collating review information, and providing administrative support. They will manage a caseload of patients identified through MDT meetings, supporting patients and carers with personalized care plans, information, training, appointment scheduling, and navigation across health and social care services.
The role contributes to reducing health inequalities, especially for individuals with long-term conditions, emphasizing independence and partnership working.

Salary: £13.50-14.50 per hour, depending on experience
Hours: 30-34 hours per week
Summary: The Care Coordinator will facilitate MDT meetings, manage patient caseloads, support personalized care planning, and liaise with stakeholders to ensure coordinated, patient-centered care. Responsibilities include scheduling meetings, taking minutes, managing referrals, supporting decision-making, and ensuring effective communication among all parties involved.
The role requires working within organizational policies, ensuring confidentiality, and traveling between sites. The post also involves ongoing training and adherence to health and safety standards.

Qualifications
* NVQ Level 2 or equivalent, with willingness to pursue NVQ Level 3
Experience
* At least 2 years in health or social care
* Experience working in multidisciplinary teams and care homes
* Knowledge of primary care, NHS issues, health and social care processes, and medical terminology
* Experience with databases and confidentiality protocols

* Willingness to undergo Disclosure and Barring Service check
