You will be supporting the Renal Social Worker in working with patients with chronic kidney disease, providing practical support to kidney patients to ensure maximum engagement with treatment plans, clinic appointments and other appropriate interventions. You will also support young adults with chronic kidney disease to help with their transition and engagement into adult chronic kidney disease services. You will assist with the delivery of patient‑centred care under the supervision of a social worker, focusing on welfare and social support as well as assistance with benefits, claims and appeals.
Responsibilities
* Work with young adults to engage them in their treatment plan, clinic appointments and other appropriate interventions to optimise and maximise their personal safety and well‑being, working holistically with other professionals where appropriate.
* Participate in young adult and transition clinics, ensuring timely support for young adults and their families and carers.
* Support renal patients with social and welfare needs, including education, employment, housing, finances and maintaining relationships.
* Advise, assist and support renal patients in making timely welfare benefit claims, appeals and challenges to improve their quality of life.
* Advise renal patients and their families about available resources and refer them to appropriate agencies.
* Assist the renal social worker with identified areas of support and provide timely and appropriate feedback.
* Identify potential risk situations in line with Safeguarding Policy and Procedures, refer these to appropriate agencies and maintain confidentiality.
* Keep the renal social worker updated on case work and any potential safeguarding concerns.
* Carry out home visits to assist patients with completing forms where appropriate.
* Maintain good communication with colleagues and agencies involved, ensuring effective communication of patients’ needs.
* Work collaboratively with other colleagues and the wider organisation to enhance the services provided.
* Maintain accurate documentation, keeping patients’ files up to date and archiving records in accordance with Trust policies and procedures.
* Use computers and technology to carry out work‑related duties, maintaining written and computerised records in line with the Data Protection Act.
* Participate in appropriate training and development activities to keep knowledge up to date.
* Attend supervision meetings with the appropriate line manager, and participate in appraisals, personal reviews and work toward agreed objectives.
* Maintain all Trust wider standards and help improve the quality of care to all who encounter renal services.
* Undertake any other duties and responsibilities within the range of grade and nature of post.
Key Requirements
* A full UK driving licence is essential.
* Successful applicants will be required to provide an immunisation and vaccination report from a UK occupational health provider or UK GP.
