Job overview
We are looking for a Band 7 Complex Case Manager to join the St Albans and Harpenden Complex Case Management Team on a fixed-term contract until March 2028. This part‑time role (22.5 hours per week) offers the opportunity to work within a dedicated and supportive team. You will job share with another Band 7 Complex Case Manager and work alongside a Band 4 Complex Case Co‑ordinator.
This is a varied and rewarding position where you will:
* Deliver high‑quality case management activities related to assessment and treatment, identifying gaps in care provision, and planning and evaluating patient care
* Support individuals with long‑term conditions and complex needs
* Contribute to the development of clinical guidelines and service improvements
* Operate in line with Trust policies and professional standards
Flexibility is required, and there may be occasions where you support teams across different bases or localities.
Main duties of the job
* Take overall responsibility for delivering effective and efficient case management in partnership with GPs, including proactively identifying and responding to changes in a patient’s condition to support their wellbeing and achieve positive outcomes
* Plan, develop, implement, monitor, and review personalised programmes of care that are preventive, supportive, and promote self‑management, including developing and maintaining crisis plans for patients and carers where appropriate
* Regularly review care packages ensuring remedial action is identified and actioned where appropriate
* Screen referrals to ensure they meet service criteria and are directed appropriately
* Conduct assessments of patients to establish their needs and coordinate multidisciplinary team (MDT) meetings or clinics to review their needs and support resolution alongside the MDT
* Work collaboratively with external partners, including Adult Community Services (ACS), Hertfordshire Partnership University NHS Foundation Trust (HPFT), and voluntary and community organisations
* Respond to concerns and complaints in a timely, respectful, and constructive manner, supporting investigations and contributing to learning and service improvement
* Ensure all care delivered aligns with relevant national standards, professional guidelines, and Trust policies
Qualifications
* Registered Nurse (RN) or Allied Health Professional (AHP)
* Evidence of other post‑registration education and training
Experience
* Recent clinical nursing or allied health professional experience negotiating and liaising within a multidisciplinary team
* Experience working with people with complex health needs
* Experience supporting students and other learners (e.g., Support Workers) and teaching others in the practice environment
* Experience in care planning and fluent in record keeping
* Experience providing holistic care that involves the multidisciplinary team
* Post‑registration experience in an NHS community environment
Skills
* Knowledge of out‑of‑hospital care and the role of the Complex Case Manager
* Ability to listen effectively and give constructive feedback to clients and colleagues
* Knowledge of reasonable response to service user needs and prioritising those needs with required urgency
* Knowledge of risks involved in working in the community
* Knowledge of management of long‑term conditions (e.g., Diabetes, Heart Failure) and an understanding of the concept of self‑management
* Literacy and numeracy skills
* Knowledge of recent government legislation and changes in community care delivery
Key Attributes
* Requirement to hold a full, valid UK driving licence and have access to a car for business purposes (unless a disability under the Equality Act 2010 applies)
* Ability to organise and participate in weekly work allocation and prioritise and delegate effectively
* Ability to communicate well verbally and in writing, including computerised documentation and data entry
