Position Overview
We are looking for an enthusiastic and motivated Health & Social Care Support Worker to join our established Reablement Team. Support adults (18+) in their own homes and community settings to regain independence, improve wellbeing, and achieve personal outcomes.
Key Responsibilities
* Support individuals at home to develop confidence, personal care skills and daily living activities, promoting maximum independence.
* Carry out prescribed clinical tasks such as physiological measurements (BP, pulse, temperature), urine testing, blood sampling, and oral hygiene (training provided).
* Record and report observations accurately in line with organisational requirements.
* Work flexibly as part of a service delivering 365 days per year.
Essential Criteria
* Level 2 in Health & Social Care and willingness to complete Level 3 within 12 months.
* Experience in health or social care settings, preferably in the community.
* Ability to follow guidelines, carry out delegated duties and respond positively to feedback.
* Good general education.
* Ability to converse in Welsh with older people about their health and wellbeing.
* Ability to write in English (Welsh writing desirable).
