About the Role We are seeking a dedicated and compassionate Social Worker to join an All‑Age Disability (0-25) service. This role supports children, young people, and young adults with disabilities that significantly impact their daily lives. You will work closely with individuals and families to identify needs, develop outcome‑focused plans, and ensure a smooth transition through different stages of life.
