If you answered yes, then come join us. We are seeking a physiotherapist to work across Redditch and Bromsgrove. The Reablement team assists patients requiring additional support and rehabilitation on discharge from hospital, and those in the community who need increased care or therapy input. Assessments are holistic, person centred, and focused on maximising independence. Patients are at the heart of everything we do.

This is a 34.5 hour post, with flexibility of hours considered for the right person (minimum of 30 hours over 4 days). For more information, please contact Colin Humpage.


Main duties of the job

As a member of the reablement team, you will work closely with therapists and social care teams, having the unique opportunity to experience both sectors whilst with us. You will perform holistic, goal focused, patient centred assessments, and develop thorough treatment programmes to meet the goals. You will manage a varied caseload, spanning all physiotherapy specialities. You will work closely with both health and social care colleagues, in a blended approach to patient care. Physiotherapists, nurses, occupational therapists, therapy assistants and social care staff undertake collaborative working on a daily basis to ensure that all patient needs are met. Regular clinical supervision and professional support are provided. We positively encourage any special interests which complement the patient demographic that we see. Job responsibilities

* Work as an autonomous practitioner within an integrated team providing timely and expert professional assessment and treatment interventions to patients.
* Provide case management for a designated caseload of patients who have immediate and/or complex care needs.
* Triage patients to ensure prioritisation of clinical need. Develop and co‑ordinate individualised rehabilitation plans in close liaison with the team, the patient and their carers as well as all other services.
* Be responsible for the timely assessment and treatment of patients who have urgent and/or complex primary physiotherapy needs.
* Co‑ordinate appropriate interdisciplinary, patient centred care that promotes independence for patients on your caseload.

Person specification


Qualifications

* Degree / diploma in Physiotherapy
* HCPC registration
* Evidence of relevant post graduate courses / Continual Professional Development maintained in portfolio.
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
* Relevant validated clinical courses.


Additional Criteria

* Able to meet the travel requirements of the role.
* Membership of the Chartered Society of Physiotherapists.


Knowledge

* A range of evidence based Physiotherapy clinical skills and knowledge appropriate to the role.
* A good level of knowledge in a wide range of physical and mental health conditions. Knowledge of rehabilitation concepts and services.
* Knowledge of research and audit processes.
* Working knowledge of Mental Capacity Act.
* Knowledge of appropriate NSFs and guidelines.


Experience

* A range of post graduate experience including work in a variety of specialities.
* Experience of caseload management and prioritisation.
* Experience of supervision of junior staff or students.
* Experience of delegation to qualified/ unqualified staff.
* Recent experience in a community setting.
* Experience at band 6.


Skills and Abilities

* Ability to challenge poor behaviour.
* A positive attitude and able to act on feedback.
* Ability to communicate clearly and effectively in English in both verbal and written formats.
* Confidence in passing on skills/ knowledge and completing teaching sessions/ presentations.
* Ability to work proactively with other agencies to prevent hospital admission and facilitate early discharge.
* Ability to use IT confidently and competently in daily work.
