Arare and exciting opportunity has arisen to become a Roald Dahl Children withMedical Complexities (CMC) Transition Nurse Specialist at Leeds TeachingHospitals NHS Foundation Trust (LTHT) in partnership with the amazing RoaldDahl Marvellous Children's Charity.

Thepostholder will be a highly motivated, compassionate, and accountableindividual who can work autonomously to improve the transition care for youngpeople with medical complexity transitioning from childrens services toadult health services at LTHT. They will share the Trust values and becommitted to equality, diversity, and inclusion.

Thecohort of children and young people we are looking to support with the newCNS roles are under the care of three or more medical specialties. Theirmedical conditions are life-long and complex, and the pressure placed ontheir parents/carers is relentless.

Thecomplexities and health needs of these children and young people with CMC canresult in fragmented, inconsistent, and uncoordinated care which often causesconfusion and unsatisfactory outcomes for them and their families.

TheRoald Dahl CMC Transition Nurse Specialist will enable proactive andinclusive services centred on the needs of the child and family. The benefitswould include a reduction in emergency admissions, prevent hospitalisation,improved communication.

Main duties of the job

Mainduties of the role:

Toprovide a high-quality CMC transition service, meeting the individual needsof children, young people, and their families.

EstablishCMC transition clinics and MDT coordination of CMC across LTHT and in thecommunity.

Providinghealthcare transition support to staff and professionals at LTHT ensuring acohesive and a co-ordinated transition process.

Coordinatecare within the hospital setting and work with community teams to coordinatehealthcare transition and improve access to the CMC transition service.

Provideclinical expertise on CMC transition to improve safe standards of care byactive involvement in the patient journey and development of patientpathways.

Workcollaboratively with all stakeholders including MDTs, CNSs, clinical teams,Consultants, AHPs, and community nursing teams across the trust and the ICBto facilitate improved patient flow, capacity, and continuity of care.

Toeffectively liaise with education and social care to reduce fragmentation oftransition care across organisational boundaries.

Provideevidence-based healthcare transition teaching/training to CYP and theirfamilies, providing up to date evidence based verbal and writteninformation/leaflets that is age and culturally appropriate to promote anddevelop their confidence to become more independent in managing their health

Job responsibilities

Thepost holder will be practising as an independent specialist clinicalpractitioner providing patient-centred and evidence-based clinical care,within an agreed scope of competence and practice. The jobholder isresponsible for the management of a defined caseload. The jobholder carriescontinuing responsibility for the assessment of care needs, the development,implementation and evaluation of programmes of care and the setting ofstandards of care.

Thejobholder will provide specialist nursing care, treatment and advice to aspecific patient group and will contribute to the development of thespeciality service. The jobholder will ensure cost effective use ofresources. The job holder will support the development of interdisciplinaryand whole service working. As a rolemodel they will promote the sharing of good practice and delivery of highquality, effective and efficient care. The job holder will provide trainingand education to staff groups, patientsand families/carers.

Thepost holder will work in partnership with the multi-disciplinary team (MDT)to implement and improve the patient pathway and will be expected to developnew skills to further enhance the quality of the service delivered. Whereappropriate, the role will include the day-to-day leadership and management.

Leadership

Provide leadership to the clinicalteam working with Matron, develop new roles/ways of working

Provide a visible clinical lead androle model to the nursing and wider team by delivering high qualitycompassionate care.

Support staff to provide efficient,effective, safe and quality care to patients as an expert practitioner.

Deliver a positive patientexperience.

Support in the delivery of aneffective, evidence based, safe, quality service within budgetary envelope.

Responsible for the day to daysupervision of junior clinical staff and students within the specialist team.

Motivate staff in the delivery ofquality patient care.

Has responsibility for the linemanagement of the specialist nursing team including recruitment, performancemanagement and workforce planning.

Deliver an effective qualityservice with budgetary constraints, monitors the delegated budget for thespecialist nursing team. Work with the Matron to work towards the Nursing,Midwifery and Allied Health Professionals objectives and service improvement.

Supervise clinical delivery of carecompetencies.

Anticipate problems and takepreventative or mitigating actions or have contingency plans available.

Investigate and respond toaccidents, complaints, untoward incidents, and other significant events.

Ensures that workload andperformance monitoring data for the team is collected, collated and utilisedto ensure the effective and efficient working of the team. Reviews thisinformation with Matron on a regular basis.

Work with the Matron in thedevelopment of specific elements of nursing practice, reflecting the CSU andNursing, Midwifery and Allied Health Professionals business plans.

Engage users and carers views onservice delivery and development.

Propose, develop and implementpolicy, protocols and practice guidelines specific to the clinical specialty.

Exercises a personal duty of carein relation to equipment, resource and stock control utilising equipment andsupplies appropriately.

Clinical

Provide high quality care as anexpert practitioner and act as a role model and resource for advice, supportand guidance.

Provides specialised advicecontributing to the diagnosis and care of patients.

Where appropriate to the servicespatient demographic, todevelopmentally appropriate, young person friendly healthcare tosupport young people; including support for transition, as they move fromchildrens into adult services between the ages 11-25 years.

Responsibility for ensuringsetting, monitoring and seeking to continuously improve standards and thequality of patient care.

Undertake clinical procedures wheredexterity and accuracy are required.

Assess, plan, implement andevaluate care and treatments using specialist clinical knowledge and clinicalskills.

Make clinical assessments andtreatment decisions within clearly defined policies and protocols.

Organises own clinical workload.

Undertake duties on wards ordepartments as and when required by service demands or patient need.

Supervise clinical staff to promotethe delivery of excellent clinical standards.

Receives supervision of clinicalcare and outcomes from nurse specialist team leader.

Act in accordance with the relevantprofessional code i.e. NMC.

Fulfil the requirements as directedby the Nursing and Midwifery Council to maintain and revalidate registration.

Ensure and promote the maintenanceof a safe, clean and secure working environment.

Act in such a way that safeguardsthe health and wellbeing of children and vulnerable adults at all times. Isfamiliar with and adheres to, the Leeds Teaching Hospitals Trust (LTHT) safeguarding policies.

Raise concerns with regards torisk, danger, malpractice or wrong doing by following the LTHT Whistleblowingpolicy and supporting processes.

Ensure that documentation standardsare maintained in accordance with NMC and LTHT guidelines, whether written orcomputerised.

Promote and maintain positiveworking relationships between all staff, internal and external to the Trust,involved in the care of patients.

Communication

Maintains accurate clinical andother records using the LTHTs documentation.

Support patients and theirfamilies, communicating sensitive condition related information, ensuringthey receive required information to enable them to participate in their caredelivery.

Deliver complex information topatients and staff.

Liaise with external healthproviders and other agencies (as appropriate) to ensure seamless provision ofcare delivery.

QualityImprovement & Research

Improve the quality and safety ofour services by being proactive in promoting and supporting the LeedsImprovement Method.

Participate in quality improvementprogrammes as required.

Keep up to date with andcommunicate service developments, advances and research.

Through the use of audit, localresearch, metrics and feedback continually seek to identify practice thatneeds improvement. Implement and evaluate any resultant improvement plan.

Actively promote patientinvolvement and feedback within service improvements to promote a positivepatient experience

Maintain and sustain the momentumof improvements until they are firmly embedded into the values and culture ofthe clinical area.

Undertake audit programmes specificto client group, treatments and developments.

Help to set, monitor and seek tocontinuously improve standards and the quality of patient care, includingthose standards defined in local and national guidance.

Participate in clinical trials asappropriate.

Work closely with Research andInnovation to promote research, disseminate outcomes and findings to supportevidence based practice.

Educationand Training

Participate in own and othersappraisals and in the development of own personal development plan andperformance review.

Provides specialist training andeducation at post-graduate level within the University settings as agreedwith Matron.

Undertake role specific trainingand competencies.

Provide education and training topatients and their carers.

Keep up to date with developments,advances within the specialty.

Collaborate with other members ofthe team in clinical areas and departments to ensure that the provision ofeducation and training is clear and robust.

Mentorship and supervision ofjunior staff.

Participate in specialist national,regional and local conferences as agreed with Matron.Support staff to deliverevidence based practice and research activities to further enhance patientcare.

Person Specification


Experience

* Significant experience of working with young people
* Significant and recent relevant experience at Band 6 or equivalent within an appropriate specialty
* Clinical audit and standard setting

Qualifications

* Registered Nurse (RN Adult for adult clinical areas or RN Child for childrens areas (Level 1) or Registered Allied Health Professional (AHP).
* Recognised teaching/assessing qualification or equivalent demonstrable experience of teaching and assessment.
* Evidence of continued professional development
* Post registration qualification in specialty or equivalent demonstrable experience.
* Master Level Degree Education or equivalent experience
* Extended/ Supplementary Prescribing
