As a Care Coordinator you will ensure high-quality, person-centred care is delivered to clients in their own homes through effective coordination of care workers, scheduling, communication, and documentation. The Care Coordinator is responsible for maintaining continuity of care, supporting staff, covering shifts if required and ensuring compliance with CQC regulations and organisational policies.

* Hours: Monday-Friday, 8am-4.30pm (required to work one weekend in four)
Key responsibilities:
Care Planning & Client Management
* Complete initial assessments, reviews, and risk assessments alongside the Registered Manager
* Develop and maintain up-to-date, person-centred care plans
* Respond promptly to changes in clients' needs and amend care plans/schedules accordingly
* Maintain clear communication with service users, families, social workers, and healthcare professionals
Scheduling & Rota Management
* Create and maintain staff rotas
* Match carers to clients based on continuity, location, skill, and compatibility
* Ensure punctuality and continuity of care by monitoring rota gaps, sickness, and lateness
* Work with the on-call system to resolve last-minute rota issues
Staff Coordination & Support
* Assist with recruitment, onboarding, shadowing, and field-based support of care workers
* Provide ongoing supervision, spot checks, and competency assessments
* Offer guidance and support for staff dealing with challenging situations in the community
Quality Assurance & Compliance
* Ensure service delivery meets CQC standards and company policies
* Monitor care delivery through audits, reviews, spot checks, and client feedback
* Record and report incidents, safeguarding concerns, medication errors, or complaints promptly
* Maintain accurate and timely documentation on digital care systems
Communication & Administration
* Update electronic systems, logs, schedules, and care records daily
* Support the Registered Manager with compliance tasks, investigations, and service improvements
* Participate in staff meetings, training sessions, and quality reviews
* Participate in an on-call rota
What we are looking for:
Essential:
* Previous coordination or supervisory experience in domiciliary care or a health and social care role
* Understanding of person-centred care, safeguarding, and CQC regulations
* Ability to multi-task in fast-paced environment
* Excellent communication and relationship building abilities
* Strong organisational and time-management skills
* Ability to remain calm under pressure and manage last-minute changes
* Compassionate, patient, and client-focused approach
* Good IT skills (rostering systems, care systems, emails, reporting)
* Ability to work independently and as part of a wider team
Desirable:
* NVQ/QCF Level 3 in Health & Social Care
* Experience creating rotas using digital scheduling systems (e.g. CarePlanner, PASS, Birdie)
* Full UK driving licence
