Support Worker in Dementia and Delirium Care
We are looking for a compassionate, enthusiastic and caring Healthcare Assistant who is reliable and flexible to join our Dementia and Delirium Team.
Role Overview
As part of our vision to provide excellence in Dementia and Delirium care across the organisation, the team offers an exciting opportunity for a Dementia Support Worker to join within Somerset Foundation Trust. Dedicated individuals who enjoy engaging with patients with cognitive impairment and who are committed to providing individualised care are sought.
The role will entail working across all inpatient wards within acute and community services to support patients and ward staff in caring for those who have dementia and are at risk of developing delirium. The successful candidate will help develop care plans, provide personalised meaningful activities, liaise with families and carers, and support discharge planning with the wider multidisciplinary team.
Key Responsibilities
* Organise and plan your own caseload and adjust care plans as situations change.
* Ensure practice always meets Trust standards and aligns with Trust policies.
* Engage in ongoing personal and professional development through courses, clinical supervision and peer supervision.
* Operate appropriate information and digital technology systems for record keeping and data analysis.
* Improve patients’ mental health, physical health and wellbeing through therapeutic activities.
* Support ward staff with management strategies for behavioural and psychological symptoms of dementia.
* Identify patients admitted with dementia or delirium and proactively contact them and their families/carers.
* Document and flag appropriate information and make appropriate referrals.
* Liaise with wards to complete ‘What matters to me’ documents meaningfully.
* Establish relationships with patients and engage their families/carers regularly.
Qualifications and Experience
* Adult/elderly rehabilitation, nursing, care worker experience.
* NVQ 2/3 or equivalent in care/rehabilitation (if not already held).

* Good verbal and written communication.
* Time management and flexibility.
* Initiative and ability to work as an integrated team member or under own initiative.
* Willingness to learn and develop new skills.
* Awareness of compassion, distressing circumstances impact.
