Transition Worker - Early Identification

Primary work location: County Hall, Taunton (remote work available with manager approval). Permanent role, 37 hours per week (32 hours per week contract also available). 25 days annual leave (excluding bank holidays, pro rata for part‑time).
What will I be doing?
As a Transition Worker you will support children and young people accessing neurodiversity pathways, gathering detailed information from new referrals to create comprehensive support plans, working closely with families and early years settings and schools.
* Gather information from new referrals to produce records and plans for young people with autism.
* Support communication of young people with autism, e.g., create symbol resources.
* Work in partnership with professionals, attend team meetings and support transition between home and school.
* Provide targeted autism telephone support and specialist calls to families and education settings.
* Deliver one‑to‑one work in family homes, offering strategies and home visits as needed.
Experience & qualifications
* Relevant qualification in education, social work, or related field.
* Training in autism awareness and support strategies.
* Certification in safeguarding children and first aid.
* Excellent communication and interpersonal skills; ability to work independently and manage a caseload.
* Strong organisational and time‑management skills, ability to create and implement effective support plans.

Salary and contract
Grade 13, ranging from £25,989 to £28,142 per annum for 37 hours per week (£22,476 to £24,339 per annum for 32 hours per week).
