An exciting opportunity has arisen for a highly motivated, enthusiastic District Nurse with a Community Nursing Specialist Qualification to take up the post of Band 7 District Nurse Team Leader within the Community with Core Clinical Services (CCCS) Division working across the Dudley Borough.

Main duties of the job

The post-holder has overall accountability for the District Nursing Team within their Primary Care Network (PCN), and responsibility for the ongoing assessment, development, implementation and evaluation of programmes of care for patients on the District Nursing caseload.

Essential criteria

1. Must be able to commute to meet the demands of the role

Experience

2. Evidence of leadership / management qualities
3. Evidence of teaching / supervision skills
4. Demonstrates up to date evidence based clinical trends in relation to community nursing
5. Experience at Band 6 in community nursing

6. Operational experience in the delivery of disease management/Long term conditions

7. Up to date knowledge of current Government and Local imperatives and how these translate into local practice
8. Computer literate – ability to use software programmes designed to maximise the contribution to the post

Multi-site working

The four partner Trusts of the Black Country Provider Collaborative (Dudley Group NHS FT, Royal Wolverhampton NHS Trust, Sandwell & West Birmingham NHS Trust, and Walsall Healthcare NHS Trust) are actively working in partnership to deliver a collective vision focused on providing "better, faster and safer care to the population of the Black Country and beyond".

To support this vision, we are (where appropriate and safe to do so) actively looking at new models of care which:

33. Better protect elective / planned care service delivery across the system by organising on a 'hot and cold' arrangement
34. Consider the optimal use of resources through consolidation and specialisation, and
35. Seek to reduce unwarranted variance, level up / standardise care through a 'Networked Service Solution' approach across the four sites

Against this context, although this post is hosted by The Dudley Group, there may be an expectation to work flexibly across the four partner Trusts to better meet patient and service needs.
