We are looking for a dynamic and patient focused Occupational Therapist to join our Community Therapy Team.
Responsibilities
* Work alongside community therapy colleagues, community nurses and matrons to provide rehabilitation, palliative care and hospital at home/unplanned therapy support to residents in Hertfordshire settings.
* Provide specialist occupational therapy intervention and support or delegate OT skills of other therapy staff.
* Manage a clinical caseload, support patients with goal setting and help achieve functional performance through activity.
* Ensure effective patient flow through the service and support triage on pathways.
* Provide expertise in equipment provision, effective therapeutic moving and handling and cognitive assessments.
* Collaborate with the MDT to help patients achieve goals and receive timely therapy input.
Qualifications and Requirements
* HCPC registered Occupational Therapist with the relevant qualification.
* Minimum of 2 years qualified experience.
* Full valid driving licence and access to a van or car for business purposes (unless disability as defined by the Equality Act 2010).
* Willingness to travel across Hertfordshire, including Stevenage and North Herts; ability to support other teams at weekends.
