The Role:
The service provides a stepping stone to independence by supporting people to regain lost skills, learn new skills, and adapt to the challenges of living independently.
You will work within a CQC “Good” rated - Home Support Reablement Council Team, collaborating with NHS colleagues, Social Workers, and Occupational Therapists to provide care including moving and handling, personal care, administering medication, and ensuring service users' dignity and wishes are central to the care provided.
Home Support Reablement is a short-term service with a quick turnover of service users. The role involves supporting people in their homes at risk of needing social care or with increased care needs, helping them regain functioning, maintain life skills, rebuild confidence, and promote wellbeing.
28-hour contracts are available on a rolling rota with flexible shift patterns, including split shifts. See attached rota examples.

Do you have a Full Driving License and access to a vehicle for work purposes? We are recruiting support workers passionate about supporting and caring for adults in the community, with the ability to work independently. You should communicate clearly with people of all levels and have skills to encourage engagement and help individuals reach their potential.
No experience is required for the role, but a commitment to working in social care is essential.
