We have a fantastic opportunity for a motivated Practice Nurse with qualifications and experience in Diabetes Management to join our friendly and dynamic team.
Main duties of the job
You will be running regular Diabetic clinics across the three practice sites as well as some routine vaccination and wound care clinics. If you have any other Practice Nurse qualifications such as smear taking or other chronic disease management we will ensure to utilise and support development of your skills and experience.

Job responsibilities
The postholder is responsible for the delivery of Diabetes care to our patient population as well as other basic practice nursing duties. Supported by the clinical team within thepractice, they will deliver care within the boundaries of their role, focusingupon supporting patients to be healthy, monitoring of long-term conditions,health prevention and screening activities. They will work collaboratively withthe general practice team to meet the needs of patients, supporting thedelivery of policy and procedures, and providing nurse leadership andmentorship as required. The post holder will also contribute to the practicesdelivery of the Quality and Outcomes Framework (QOF).

Qualifications
* Nursing and Midwifery Council (NMC) approved degree in nursing.
* Diabetic management.
