About the role as a Family Outreach Worker:
We are seeking a proactive, compassionate, and highly organised Family Outreach Worker to play a fundamental role during the first-year development of our brand-new Post-16 specialist EHCP Hub in Wokingham. The centre is designed to support young people with complex needs as they transition into adulthood.
You will work closely with the Centre Manager and Wokingham Local Authority, you will support learners and families through assessment, admissions, and transition, ensuring each young person’s voice, strengths, and aspirations are central to planning.
You will be joining at a pivotal time, helping to build and define a new specialist provision that will support young people to thrive.
This is a part time (4 days per week), fixed term contract (until August 2026 with the potential to be extended) based within Wokingham and surrounding area.
What do you need to be a successful Family Outreach Worker at Activate Learning?
1. Experience working with young people with SEND and/or their families
2. Understanding the need for a student centred approach and ability to offer varying support strategies to cover diverse learning needs
3. Minimum GCSE English and Maths grade C or above
4. Strong communication and relationship-building skills
5. Understands EHCP processes, transition planning, and multi-agency working
6. Organised, reflective, and motivated to help shape a new service
This role involves regular travel across Wokingham and surrounding areas and will include lone working, such as home visits and off-site meetings.
