Providing practical and emotional support, including welfare calls, shopping, and home visits. Supporting adults after a hospital stay to help them recover safely at home. Responding to referrals from clinicians and community health professionals. Assessing needs and following a personalised support plan for each service user. To be a successful Support Worker, what will you need? A kind, calm and confident approach with great people skills. The ability to work independently and as part of a team. A full UK driving licence and willingness to drive service vehicles. Basic IT skills and a commitment to following procedures. Interested?
