We are seeking a highly skilled Occupational Therapist or Physiotherapist to fulfil a clinical lead role within the Community Assessment Team. This is a 7-day service located in the Emergency Department of Gloucestershire Royal Hospital (GRH) and Cheltenham General Hospital (CGH).
We are looking for a therapist who demonstrates strong team skills, is capable of assessing and treating the functional needs of patients within the challenging environment of Emergency Departments and has excellent management skills. We welcome applicants who share our passion for providing a responsive service which can meet the needs of our patients in a flexible and professional manner.
Applicants must have a minimum of four years post graduate experience and extensive experience/knowledge of patient conditions pertinent to working with predominantly older adults. You will have a knowledge of community resources and pathways in Gloucestershire. In addition, you will be able to demonstrate excellent communication and organisational skills.
Main duties of the job
We run a 7-day service providing Frailty assessments to older adults located in the Emergency Department of Gloucestershire Royal Hospital (GRH) and Cheltenham General Hospital (CGH) and the Frailty Assessment Unit located at GRH. This exciting Band 7 opportunity will ensure timely and specialist assessment, treatment and discharge of older adults from the acute trust.

Qualifications
* Registered Professional - HCPC
* Evidence of relevant post graduate diploma/short courses at Masters equivalent level.
* Evidence of continued professional development
* PADRAP, Non-Medical Prescribing.
Experience
* Minimum 4 years post registration experience
* Experience of providing acute and community patient care
* Expert knowledge of community discharge pathways and how to access them
* Experience of leading, teaching and supporting colleagues.
